Das Bauwerk ist eine dreigeteilte, symmetrische Anlage. Der Mittelbau wurde in Form einer schlichten Friedhofskapelle mit Dreiecksgiebel und Säulenportal errichtet. Das Mausoleum besteht im Inneren aus einem runden Vorraum und einem quadratischen Altarraum.
Der Sarkophag von Prinzessin Elisabeth steht unter der Rotunde. Er besteht aus Marmor, mit einer lebensgroßen Plastik des Kindes; und stammt von dem Bildhauer Christian Daniel Rauch. Der Sarkophag wurde im Jahre 1831 vollendet.
In den Jahren 1869/70 wurde das Alte Mausoleum nach Plänen von Professor Heinrich Wagner erweitert. Die beiderseits angebauten, symmetrischen Flügel wurden zusammen mit den zwei seitlichen Grabkapellen, die über Kolonaden mit dem Mittelteil verbunden sind, im Stil an Mollers Architektur angepasst.
In der nördlichen Grabkapelle ruhte ursprünglich Großherzogin Alice, die Mutter des letzten Großherzogs Ernst Ludwig, der seine Eltern im Jahre 1910 in das Neue Mausoleum umbetten ließ. Heute ruht in dieser Grabkammer Prinz Emil.
In der südlichen Kapelle ruhen die Brüder Großherzogs Ludwig IV., die Prinzen Wilhelm und Heinrich.
Ebenfalls im Alten Mausoleum begraben liegen der erste Großherzog, Ludwig. I, und seine Gattin, Großherzogin Luise. Zusammen mit den Prinzen wurden ihre Särge im Jahre 1910 von der landgräflichen Gruft in der Stadtkirche in das Alte Mausoleum überführt.
