Park Avenue Community Health Center

I went to the health center for medical care at the Park Avenue location. Although staff were friendly, I needed referrals for my medical conditions, and provided specific contact information from the facility waiting to receive the referral. I did this on several occasions, called, and visited repeatedly, but kept being told that they will send a referral for several weeks at a time. The pharmacy tried reaching the doctor, or front desk staff by phone to facility to get prior authorization, but couldn't get a return call. I tried reaching out to the doctor, and nurse, and front desk staff concerning this prescription, but was unsuccessful due to lack of return calls, and lack of staff member answering the phone in the medical clinic. Staff members went on vacation, and still no one responded during my serious medical need, even despite the social worker advocating to the medical team, and sending emails to the medical team, and supervisors etc. I was saddened by this lack of follow up, as I did not want to have to run all over town and change to a different primary care doctor, nor did I want to have my medical conditions advance due to serious neglect, and lack of follow through despite multiple attempts to obtain referrals, needed medicine, not even return call to myself nor the social worker who works within the same facility. Sad, Sad, Sad!!

Don't go thinking you can have a Covid test without being a client; even if you're a member of the community. I will never donate to this agency again.

Communication depends upon what person is assigned to your case. Israel Rivera was helpful bu I’m in a wheelchair, so why didn’t my packet go out to apts that are accessible? Leslie Campbell called me and spoke to me face to face about accesible housing and also said that 3 opportunities were available for me and my condition. I guess I have to wait because according to her “she’s in a training” so she cannot forward me appointments to meet who is looking forward to housing me

I just started their Housing program.The staff seem really nice.I attend about 2 groups a day.The groups are interesting.I was told to be consistent and housing will be available when the time comes. Keeping focus and consistent. Kevin K

They’re communication skill with working with their clients SUCKS they really don’t assist their client unless they are i life threatening situations they neglect your mental an emotional health an neglect there to keep up to date with new information with the clients I’ve been living in an Bailey house apartment for well over a year now an the ceiling has been leaking molding an cracking an when told it still has yet to be repaired also I’m a domestic violence survivor an wasn’t supposed to be moved to a Bronx unit an do you know that although they new that it would be detrimental to me an my family they still stuck me in a well INCOMPLETE apartment not to mention they is no surveillance mail boxes get broken into an when you let the staff know it just falls on deaf ear I’m literally scared for my life

Didn't take long for housing!!! Got a 4bdrm in only, 1 month!!!

I've been coming here for years. I only have issues with the Psychiatrist, the main person in charge of the Therapist. Food pantry is good.
