Dirty place,small rooms with 4 residents in the same rooms, no privacy.the elevator is old with mold on the ceiling.in addition the entrance doesn't reflect the state of the floors

Sonny is an excellent social worker. He was attentive the whole time and did an amazing job planning for discharge.

I was placed here for rehabilitation after surgery. I would like to give a huge thanks to Wendgi the wound care nurse she put her all into helping my wounds heal and for that I am so grateful! The entire rehabilitation staff is great! My therapist Joey and Mona were a huge part of my healing process. They were extremely encouraging, kind and patient with me. I really appreciate them and everyone else in that rehab room that is what I’ll miss most of all when I go. I came in unable to walk at all now I’m standing on two feet with my device of course and it’s all thanks to them. Thanksss again you all for your support!!

The most despicable and ignorant institution that I have ever had the displeasure of dealing with. My mother, Daniela Pantur, was a resident there for about a year before she passed away in that miserable place. The entire time I was going to see her there I was appalled by the conditions of this place. Every week it was the same story: Unkempt hair, nails, and dirty clothes that they kept her in, unbathed for probably a week at a time. Items of hers that I brought would often get lost and no one seemed to have a grip or idea as to where it would be. Trying to be on the phone with these people is an impossibility. When they said they would call me back about an issue I had, no calls ever came. No accountability, and most of the staff there carried themselves with no passion, no purpose. I would see them shoving food in my mother's mouth and spilling food on her, leaving those dirty clothes on her after. I also heard an interaction between a resident and worker where the resident wanted to use the restroom and could not get help- what stuck out to me was this resident saying "I don't want to go in my pants". As if they were in a rush just to move on to the next resident like it was a minute task. My mother even contracted COVID-19 in that place- how such a thing could happen is beyond me. Goes to show you the unsanitary conditions they made their residents beholden to. As I was gathering her belongings after her passing the process of trying to get in touch with anyone who could help me was dreadfully slow--and they seem to have no idea to track if staff showed up to work on some days. DO NOT LEAVE YOUR LOVED ONES HERE IF YOU CAN HELP IT. All they seem to care about is getting these poor people in the door and beyond that they can't be bothered to properly look after them.

I jave been asking since early June that the AC in my moms room isn't working. It has been reported by me several times as well as the nurses I have spoken to and nothing has been done. Two times now I have seen the maintenance man who claims it works or has fixed it and then today says will come right up and he has lied. It is 90 degrees outside and the ac only blows weak, hot air. What else needs to be done to get this fixed??????

Very welcoming and inviting staff. Attentive and calm environment. Clean with renovations going on . Enjoyed my visit with loved one .Looking forward to the next visit. Hopefully renovations will be finished. Also looking forward to my family to exit and return home soon .

The staff is helpful and organized. I was explained what my mother needs are, and was given the option to take care of her laundry since things get lost when the facility does it. My mother who is resident feels happy and relaxed.
